Fat Pad Augmentation For Foot Pain
Restoring the Foot’s Natural Cushion: How Dr. Alan Shih Uses Leneva to Help Patients Find Lasting Relief
Every step places significant pressure on the feet, and over time the body’s natural cushioning can begin to wear away. As the protective fat pad beneath the foot becomes thinner, even routine activities such as walking, exercising, or standing for extended periods can become painful. For many people, this gradual loss of cushioning has traditionally left them with few effective long-term treatment options.
Board-certified podiatrist Dr. Alan Shih, founder of Head to Toe Healthcare, has made it a priority to identify treatments that address the underlying problem rather than simply masking symptoms. One of the newer technologies he has incorporated into his practice is Leneva, an injectable treatment designed to encourage the body to rebuild its own natural cushioning beneath the foot.
Historically, patients experiencing fat pad atrophy have relied on supportive shoes, custom orthotics, gel inserts, and padding to reduce discomfort. While these measures can decrease pressure in painful areas, they do not replace the tissue that has been lost. As a result, many patients continue to experience ongoing discomfort despite wearing supportive devices.
Another option has been the use of cosmetic dermal fillers. Although these products can temporarily provide additional padding, they are gradually absorbed by the body, often requiring repeat injections within several months. The recurring treatments can become expensive, and some patients are disappointed by inconsistent results or temporary side effects such as swelling or discomfort.
Autologous fat grafting has also been used to restore cushioning by transferring fat from another area of the patient’s body into the foot. While this procedure may produce longer-lasting results, it requires surgery, creates additional recovery time, and introduces the complexity of harvesting fat before it can be implanted.
Seeking a less invasive alternative, Dr. Shih turned to Leneva.
Rather than functioning as a traditional filler, Leneva is derived from donated human adipose tissue and serves as a regenerative framework. After it is injected into the painful area, the material acts as a scaffold that supports the body’s natural healing response, encouraging the gradual development of new fatty tissue. As the body’s own tissue grows into the area, the foot regains additional cushioning that can improve comfort during daily activities.
The procedure is typically performed in the office using local anesthesia, allowing patients to avoid the risks and recovery associated with surgery. Over the following several weeks, the regenerative process continues as new tissue develops, with many patients noticing progressive improvement in comfort between six and twelve weeks after treatment. Depending on the individual, the benefits may last for several years.
For Dr. Shih, adopting new technology is less about offering the latest procedure and more about providing meaningful solutions that improve patients’ lives.
“My goal has always been to help people stay active and comfortable,” Dr. Shih explains. “When existing treatments weren’t providing the long-term results many patients were looking for, I wanted an option that restored function instead of simply providing temporary relief. Leneva gives us another way to accomplish that without surgery.”
Most patients experience minimal downtime following the injection. Although limiting excessive weight-bearing for a short period is recommended to protect the treatment area, many individuals are able to resume their normal routines relatively quickly while the regenerative process continues.
As regenerative medicine continues to expand within podiatry, treatments like Leneva are changing how physicians approach chronic foot pain associated with fat pad loss. Instead of relying solely on temporary cushioning or invasive surgical procedures, physicians now have an option that works with the body’s own healing mechanisms to restore natural protection beneath the foot.
For patients whose quality of life has been affected by painful thinning of the foot’s natural padding, Dr. Shih believes restoring that cushioning can make a meaningful difference. By combining minimally invasive treatment with regenerative technology, Leneva offers a promising long-term option for helping patients walk farther, stand longer, and return to the activities they enjoy with greater comfort.
